Medical Records
The safety of your health information is our top priority. Occasionally, patients may need to access their protected health information. Please read the criteria below for how to request medical records.
- To have records sent to another health care provider, please contact that provider directly. Their office will reach out to us for the minimal amount of information necessary to send for your care.
- To have records sent to a non-health care related entity (i.e. insurance, attorney), the entity must request your records from us in writing with appropriate medical record release documentation signed by you.
- Your records with OakLeaf Clinics - Willow Creek Women's Clinic are available to you via MyChart. With assistance setting up your MyChart, please contact our clinic at 715-832-9292.
- If you personally need to request records prior to August 1, 2025, log in to your AdvancedMD account. If your account is no longer active, please call the clinic at 715-832-9292 to request appropriate records.
Patients have the right to access their medical records. To respect our patient's rights and confidentiality of protected health information, parents and legal guardians must complete an Authorization for Disclosure of Health Information when requesting health records. Health care records are the property of OakLeaf Clinics - Willow Creek Women's Clinic and our maintained by our clinic for the benefit of our patients. Please note that we follow state and federal guidelines for time-based retention of medical records.
